Hello! I’m Jazmine and it’s a pleasure to be a part of the Self Determined Futures Team.
I hold a B.A. in Psychological Science from the University of California, Irvine. My academic background, combined with my hands-on experience, has fueled my commitment to social welfare, harm reduction, and supporting underserved communities.
I began my professional journey as a behavioral interventionist, working with individuals with intellectual disabilities. That role taught me the importance of empathy, patience, and individualized support—values that continue to guide my work today. I continued my professional journey as a case manager providing intensive case management services to high-acuity clients who have experienced chronic homelessness. I worked closely with each individual, empowering client’s choice, to develop personalized short- and long-term goals. Through this work, I’ve discovered a deep passion for leadership, teamwork, and advocacy for mental health and community well-being.
Guided by my values of community, compassion, and resilience, I continue to seek opportunities where I can make a meaningful impact. At Self Determined Futures, I’m committed to improving lives through a person-centered approach, placing the individual’s goals, values, and preferences at the heart of all support and planning.
