OUR MISSION
The mission of Self Determined Futures (SDF) L.L.C. is to empower and uplift disadvantaged and marginalized populations by providing comprehensive services and support. We are committed to addressing the unique challenges faced by these communities and working towards creating a more equitable society.
Our primary focus is on enhancing service access to disparity populations and the ability to have equitable essential resources, such as education, healthcare, housing, employment, transportation, and legal assistance. Self Determined Futures provides accessible services in seven regional areas of California: Los Angeles County, San Diego County, Imperial County, Riverside County, San Bernardino County, Ventura County, and Santa Barbara County.
Through collaborative partnerships and innovative programs, we strive to break down systemic barriers, promote social justice, and foster self-sufficiency. At Self Determined Futures, we prioritize inclusivity, respect cultural diversity, and ensure that our services are culturally sensitive and responsive to the needs of the populations we serve.
Message From Our Founder
From our humble beginnings to the present day, our mission has remained steadfast: to empower individuals to create their own paths, shape their destinies, and realize their full potential. Today, I am proud to see how far we have come in fulfilling this mission, but I am even more excited about the journey that lies ahead.
In a world filled with uncertainty and challenges, our commitment to fostering self-determination has never been more important. We have witnessed firsthand the profound impact that empowerment can have on the lives of those we serve. Whether it's through our regional vendor programs, awarded grant projects, or Independent Facilitator services for individuals entering SDP we are making a difference in the lives of individuals across California.
As we look to the future, we must also recognize that our work is far from finished. There are still countless individuals who are struggling to break free from the constraints of circumstance, who are searching for the tools and support they need to chart their own course. It is our responsibility, our duty, to stand beside them on this journey, to offer guidance, advocacy, and unwavering support.

WHAT WE DO
Coordinated Career Pathways (CCP)
The Coordinated Career Pathways (CCP) program at Self Determined Futures LLC helps individuals with developmental disabilities transition into competitive integrated employment (CIE). Over 18 to 24 months, participants receive personalized career planning, exploration, training, and job placement support.
INDEPENDENT LIVING SKILLS (ILS)
Our ILS program empowers adults with intellectual or developmental disabilities to enhance their independence through tailored training and support, whether they seek to live alone or with others, autonomy, encompassing a wide range of services from advocacy and self-advocacy to daily living skills, community integration, and safety measures. Our ILS program is customized to accommodate each individual's cultural background, and preferences.
Housing Access Service (HAS)
Our Housing Access Services program assist in finding affordable housing and achieve independent living. We provide personalized support through tenant screening, housing assessments, custom support plans, application assistance, utility setup, safety issue resolution, and move-in logistics. After securing housing, we offer ongoing support to ensure long-term success by addressing behaviors, educating tenants and landlords, fostering positive relationships, resolving disputes, preventing eviction, assisting with recertification, and training in household management and lease compliance. COORDINATED FAMILY SUPPORT (CFS)
The CFS program provides essential support to adults with intellectual or developmental disabilities living with their families, assisting with various aspects such as finding medical professionals, scheduling appointments, navigating government programs, and personalized service plans to address individual challenges and goals, all delivered with cultural sensitivity and adaptability.
SELF DIRECTED SUPPORT/INDEPENDENT FACILITATOR (099)
With our caring and professional staff we provide comprehensive support, assisting individuals in making informed budget decisions, accessing services according to their individual program plan, addressing immediate and long-term needs, participating in person-centered planning, and acquiring necessary services and supports.
