The CFS program was designed for adults that have intellectual/developmental disabilities that wish to reside with their families, and don’t want to live on their own. This program is an additional support for families and individuals that may need assistance and guidance with finding doctors, therapists, scheduling appointments, navigating and applying to government programs (Medi-Cal, Cal-Fresh, etc.), staying organized, and much more! 

 

SDF will develop a personalized service plan aimed at overcoming obstacles to remaining in the family home, addressing specific challenges, and achieving goals. Services will be delivered in a manner that honors the language and culture of individuals and their families. Additionally, services will be customized, adaptable, and responsive to the evolving needs and preferences of individuals.
