I am honored to be part of the Self Determined Futures team as a Support Services Coordinator, where I support individuals and families through the Community First Services (CFS) and Independent Living Services (ILS) programs. I am passionate about empowering individuals to achieve greater independence, access meaningful resources, and live fulfilling lives within their communities.

With more than six years of experience in patient care, service coordination, and client support, I have worked in both home care and clinical settings, assisting individuals with a wide range of needs. Throughout my career, I have been dedicated to providing person-centered support, advocating for clients and their families, and helping connect individuals to services that enhance their quality of life.

My professional experience includes care coordination, medical documentation, medication support, and collaboration with healthcare providers, community organizations, and multidisciplinary teams. I believe that effective communication and strong partnerships are essential to ensuring individuals receive the support they need to reach their goals.
