CFS/CCP Team Lead

Hello everyone, my name is Jocelyn, and I am thrilled to be part of such meaningful work here at Self Determined Futures. I feel grateful to be part of an incredible team dedicated to making a difference.


I hold a Bachelor's degree in Sociology from the University of California, Santa Barbara, where I studied complex and underrepresented populations. After graduation, I worked as a Behavioral Technician alongside clinicians, supporting individuals in overcoming barriers to achieve their goals. This experience provided me with valuable insights into the realities faced by disabled individuals and allowed me to apply my knowledge in a practical setting.


Following that, I served as a case manager for a non-profit organization specializing in homeless services. In this role, I worked with underrepresented communities, including those dealing with substance abuse, physical disabilities, and mental health challenges. I utilized trauma-informed care and harm reduction approaches while assisting individuals in navigating government assistance programs, mental health services, recovery processes, and societal integration.


These experiences have led me to my current role as the Team Lead for the CFS/CCP Gardena team at Self Determined Futures. I’ve come to realize that many individuals are unaware of the resources available to them. In my current position, I have the opportunity to bridge that gap by connecting people in need with the resources that can support them and their families. Working with individuals who are intellectually and developmentally disabled allows me to apply all my previous knowledge while continuing to learn and grow in the process.


As a Team Lead, I aspire to lead by example with an open mind and to serve as a strong support system for both my team and the individuals we serve. I am committed to making a positive impact and helping others access the resources they deserve!
