JCOD Care Management (JCM)
Care Management for Justice-Involved Individuals.
We provide Standard Care Management (SCM) and Enhanced Care Management (ECM) to individuals who are currently incarcerated, being released from jail, or navigating re-entry into the community. Our goal is to help each person rebuild stability, access the resources they need, and successfully reintegrate — reducing the likelihood of returning to the justice system.
Our team works one-on-one with participants to identify their needs and connect them to the right support at the right time, including:

Health & Wellness — physical and mental health care coordination, substance use disorder services

Stability & Basic Needs — enrollment in social services, housing support

Employment & Education — job readiness, training, and education connections

Personal Growth — cognitive behavioral programming, arts and entrepreneurship opportunities
Standard Care Management offers this same person-centered navigation and support to individuals who don't meet Medi-Cal's Enhanced Care Management eligibility criteria, ensuring no one is left without help based on eligibility alone.
Enhanced Care Management is a Medi-Cal benefit for individuals transitioning from incarceration who have a qualifying condition — such as a serious mental illness, substance use disorder, chronic health condition, intellectual or developmental disability, traumatic brain injury, HIV/AIDS, or pregnancy/postpartum status. ECM coordinates care across medical, behavioral health, and social service systems for a smoother, more supported transition home.
Who we serve: Adults with a history of justice-system involvement — including those who are incarcerated, on probation, or on parole — who are ready to take voluntary steps toward a healthier, more stable future.
