Greetings, my name is Jocelyn Hernandez and I am very grateful to be a part of Self Determined Futures as a Coordinated Family Support Case Manager. My journey in healthcare is driven by a deep, lifelong passion for helping those in need and making a meaningful difference in people's lives. I hold a Bachelor’s Degree in Health Science from California State University, which laid the academic foundation for what has now become over nine years of dedicated experience in healthcare administration and case management. Being bilingual in English and Spanish has allowed me to act as a vital bridge in our community, ensuring that diverse populations feel heard, supported, and cared for.

Beyond my professional background, my natural instinct to care for others extends straight into my personal life. I am a massive animal lover, and my home is filled with the chaos and love of five incredible dogs who keep me grounded and constantly bring joy to my days. Whether I am advocating for a client, collaborating with my team, or caring for my pets, I bring that same core empathy and dedication to everything I do. I am thrilled to bring both my professional experience and my heart for service to this role, and I look forward to growing alongside such a wonderful team and organization.
